       b.    Resolution Authorizing Investment in LGIP (Only for new investors to the LGIP)

2. Complete required fields

3. Email completed form and backup to LGIP@aztreasury.gov​

        a.    For assistance, call (602) 542-7834

4. Investment accounting representative will process request

5. Processing time; 3-5 business days

6. Investment accounting representative will provide account number upon completion
